Long-term Management of Immunosuppression After Pediatric Liver Transplantation: Is Minimization or Withdrawal Desirable and/or Possible?
نویسنده
چکیده
Purpose of Review—The aim of this review is to review available data regarding the risks and benefits of indefinite immunosuppression versus attempted immunosuppression withdrawal for children who have undergone liver transplantation. Recent Findings—Emerging data suggest that conventional immunosuppression practices may well be responsible for a substantial proportion of the long-term mortality and morbidity burden borne by pediatric liver transplant recipients. The cumulative risk of chronic kidney disease, infection, malignancy, and cardiovascular risk factors such as hypertension, diabetes, and hyperlipidemia appear to threaten the health and well-being of children more than that of acute or chronic allograft rejection. In parallel, single center experiences have suggested that gradual immunosuppression withdrawal can be done safely with higher success rates in pediatric compared to adult liver transplant recipients. The coalescence of these two data streams has engendered substantial interest in systematic exploration of the safety and efficacy of immunosuppression withdrawal in conjunction with a vigorous scientific effort to elucidate an immunologic signature predictive of succesful withdrawal. Summary—There is a concerted effort within the transplant community to identify biomarkers that can accurately predict the success of immunosuppression withdrawal after liver transplantation. Freedom from lifelong immunosuppression is likely to yield considerable benefit, particularly for children who face the longest lifetime horizons.

Pediatric Liver Transplant
The goals of post-transplant management are to manage and treat postoperative complications, and develop a balanced long-term immunotherapy regimen that minimizes infection and side effects but controls rejection. While modern immunosuppressant regimes have reduced rates of graft loss due to rejection, they impart major risks for infection, growth failure, metabolic complications, and malignanc...
متن کاملImmunosuppression minimization vs. complete drug withdrawal in liver transplantation.
Despite the increase in long-term survival, liver transplant recipients still exhibit higher morbidity and mortality than the general population. This is in part attributed to the lifelong administration of immunosuppression and its associated side effects. Several studies reported in the last decades have evaluated the impact of immunosuppression minimization in liver transplant recipients, bu...
متن کاملRole of Immunosuppression Minimization Protocols in Renal Transplantation
The subject of transplant immunosuppression has generated significant interest in recent years. Excellent immunosuppression, advances in surgical technique, post-transplantation care, and infection control have resulted in excellent outcomes. There is widespread support for the notion that the fundamental objective in transplant immunology should be the achievement of specific graft toleran...

Background: Pediatric liver transplant is a complex surgical procedure. Pediatric liver transplantation has evolved over the last two decades into an effective and widely accepted therapy for infants and children. The aim of our report is to review the current state of knowledge and explore the incidence of complications, the involved pathogens among liver recipients to determine the effective ...
متن کاملWithdrawal of Immunosuppression in Pediatric Liver Transplant Recipients in Korea
PURPOSE We identified pediatric liver transplant recipients with successful withdrawal of immunosuppression who developed tolerance in Korea. MATERIALS AND METHODS Among 105 pediatric patients who received liver transplantation and were treated with tacrolimus-based immunosuppressive regimens, we selected five (4.8%) patients who had very low tacrolimus trough levels. Four of them were noncom...
